Arterial switch operation — transposition of the great arteries

Anatomical repair of transposition in the newborn, with coronary transfer and repair of an associated VSD or arch obstruction where present. This page is for colleagues considering a referral.

Newborns with transposition and an intact ventricular septum should be discussed in the first days of life, so that timing can be planned. For an urgent case, call rather than email.

1

Cases considered

  • Transposition with an intact ventricular septum (neonatal)
  • Transposition with a ventricular septal defect
  • Transposition with aortic arch obstruction
  • Late presentation — assessment of left ventricular readiness for a switch
2

What to send

  • Echo: coronary pattern if seen, septal defects, the arch and left ventricular geometry
  • Saturations, prostaglandin infusion, and whether a balloon atrial septostomy was done
  • Gestational age, age and weight
  • Ventilation status and current medication
  • A one-paragraph clinical summary
3

What comes back

  • A written opinion on operability and timing
  • Proposed date of surgery
  • The hospital suited to the case
  • A named onward referral if the case falls outside this service
