Ross and Ross–Konno procedures

Aortic valve replacement with the patient’s own pulmonary valve (the pulmonary autograft), combined with Konno enlargement of the left ventricular outflow tract when that is small too. This page is for colleagues considering a referral.

Cases considered

  • Congenital aortic stenosis or regurgitation in children, adolescents and young adults, where repair would not be durable
  • A small aortic annulus or tunnel-type subaortic obstruction that needs root enlargement (Ross–Konno)
  • Failure of balloon valvotomy or of an earlier aortic valve repair
  • Patients in whom lifelong anticoagulation should be avoided

What to send

  • Echo report with annulus, root and outflow-tract measurements, gradient and regurgitation grade
  • An assessment of the pulmonary valve
  • Reports of any previous intervention or operation
  • Age, weight and current medication
  • A one-paragraph clinical summary

What comes back

  • A written opinion on the approach — Ross, Ross–Konno, or an alternative such as the Ozaki procedure or repair
  • Proposed timing
  • The hospital suited to the case
  • A named onward referral if the case falls outside this service
